In order to qualify as an orphan or ward of the court for federal financial aid purposes you must meet certain specific criteria. Please answer the following questions and provide the requested documentation in order for us to determine your status.

______ 1. I am currently a ward of the court.

(Please attach a copy of the court order.)

______ 2. I am currently over the age of 18, however I was a ward of the court until age 18.

(Please attach a copy of the court order)

______ 3. Both of my parents are deceased and I do not have an adoptive parent. Please explain

where you lived and how you supported yourself following your parents’ deaths.

(Please attach copies of both parents’ death certificates).
