Unusual Enrollment History Form

Cayuga Community College Financial Aid Office has received your application for federal student aid for the 2014-2015 academic and the United States Department of Education has flagged your application for having an “unusual enrollment history”. 

 

In order to continue processing your financial aid (including student loans), you must:

 

·         Provide academic transcripts to the Registrar’s Office from all of the colleges you have attended in the past three years (2011-12, 2012-13, and 2013-14).  The transcripts can be an official copy or an unofficial copy.

·         Once you have requested these transcripts, please notify the Financial Aid Office.

·         Please list the name of the colleges you have attended in the past three years.

If your transcript(s) fail to show sufficient earned credit hours you will need to:

 

·         Provide a written explanation of why you were not able to earn credit hours and the reason you have attended multiple colleges during the past several years.  Third party documentation may be required to support your claims.

 

Once the above information has been received your application will be reviewed by the Financial Aid Office and you will be notified of your federal aid eligibility.  Failure to submit the required documents may lead to loss of all federal aid.
